What is Allergic Rhinitis (Hay Fever)?

Allergic rhinitis, commonly known as hay fever, is a group of symptoms affecting the nose, eyes, and throat. These symptoms occur when your immune system overreacts to something in the environment that is typically harmless to most people, such as pollen, dust mites, mold spores, or pet dander. Your body mistakenly identifies these allergens as dangerous invaders and releases chemicals, including histamine, into your bloodstream to fight them off. This histamine release is what causes the classic, frustrating symptoms of allergies.

Seasonal vs. Perennial Allergies

Allergic rhinitis is generally classified into two categories, depending on when your symptoms occur:

  • Seasonal Allergic Rhinitis: Symptoms only occur during specific times of the year when certain plants pollinate. This is usually triggered by tree pollen in the early spring, grass pollen in the late spring and summer, and weed pollen (like ragweed) in the late summer and fall. Mold spores, which thrive in damp, rotting leaves, can also trigger seasonal allergies in the fall.
  • Perennial Allergic Rhinitis: Symptoms occur year-round and are typically triggered by indoor allergens. Common culprits include dust mites (microscopic organisms that live in bedding, carpets, and upholstery), pet dander (flakes of skin shed by cats and dogs), indoor mold, and cockroach droppings.

Common Symptoms to Watch For

Unlike a common cold, which usually develops gradually and resolves within a week to 10 days, allergy symptoms start immediately after exposure to the allergen and can persist for weeks or months—as long as the exposure continues. Common symptoms include:

  • Frequent, repetitive sneezing
  • A runny nose, usually producing a thin, clear discharge
  • Severe nasal congestion or a "stuffy" feeling
  • Itchy, red, or watery eyes (allergic conjunctivitis)
  • Itching in the nose, roof of the mouth, or back of the throat
  • Postnasal drip, which can cause a chronic cough or a sore, scratchy throat
  • Fatigue, irritability, and difficulty concentrating due to poor sleep

Effective Management and Treatment

The most effective way to manage allergic rhinitis is to identify and minimize your exposure to the specific allergens triggering your symptoms. This might involve keeping windows closed during high pollen seasons, using high-efficiency particulate air (HEPA) filters in your home, washing bedding weekly in hot water to kill dust mites, and keeping pets out of the bedroom.

However, complete avoidance is rarely possible. When symptoms strike, several medical treatments can provide significant relief:

  • Oral Antihistamines: Over-the-counter medications like cetirizine (Reactine), loratadine (Claritin), or fexofenadine (Aerius) block the effects of histamine, reducing sneezing, itching, and a runny nose without causing significant drowsiness.
  • Decongestants: Available as pills or nasal sprays, these shrink swollen blood vessels in the nasal passages to relieve stuffiness. Nasal spray decongestants should not be used for more than three consecutive days to avoid a "rebound" effect.
  • Corticosteroid Nasal Sprays: These are the most effective medications for treating the severe nasal inflammation and congestion caused by allergic rhinitis.
