Understanding Acne Vulgaris

Acne, medically known as acne vulgaris, is the most common skin condition worldwide. It occurs when your hair follicles (pores) become plugged with oil and dead skin cells. While it is most prevalent among teenagers undergoing hormonal changes during puberty, acne can affect people of all ages, including adults experiencing hormonal shifts, stress, or using certain medications or cosmetics.

The Root Causes of Acne

Acne is a complex condition driven by four main interacting factors:

  • Excess Sebum Production: Sebaceous glands in the skin produce an oily substance called sebum. Overproduction of sebum, often triggered by hormonal fluctuations (like androgens), creates an environment ripe for acne.
  • Clogged Pores: Dead skin cells, instead of shedding normally, clump together with the excess sebum and plug the hair follicle, creating a microcomedone.
  • Bacterial Overgrowth: Cutibacterium acnes (C. acnes) is a type of bacteria that naturally lives on the skin. When a pore becomes clogged with oil, these bacteria rapidly multiply inside the blocked pore.
  • Inflammation: As the bacteria multiply, your immune system responds by sending white blood cells to the area, resulting in the red, swollen, and painful bumps characteristic of inflammatory acne.

Types of Acne Lesions

Acne is not a single type of blemish. A proper treatment plan depends on identifying the specific types of lesions present:

  • Non-Inflammatory Acne: This includes closed comedones (whiteheads) and open comedones (blackheads). The black color is not dirt, but oxidized melanin exposed to the air.
  • Inflammatory Acne: This includes papules (small red bumps) and pustules (bumps filled with white pus).
  • Severe Nodulocystic Acne: Deep, painful, solid lumps (nodules) or pus-filled cysts beneath the surface of the skin. This severe form carries a high risk of permanent scarring and requires specialized medical intervention.

Effective Skincare and Prevention

A consistent, gentle skincare routine is the foundation of acne management. Wash your face twice daily with a mild, non-comedogenic (won't clog pores) cleanser. Avoid harsh scrubbing, which can irritate the skin and worsen inflammation. Always remove makeup before going to sleep, and ensure all moisturizers, sunscreens, and cosmetics are oil-free and labeled non-comedogenic.

Resist the urge to pop, squeeze, or pick at pimples. This forces infected material deeper into the skin, increasing inflammation, prolonging healing time, and significantly raising the risk of permanent scarring and dark spots (hyperpigmentation).

Professional Treatment at Ultima Pharmacy

If over-the-counter products containing salicylic acid or benzoyl peroxide aren't clearing your skin, stronger medications are needed. Under the minor ailments program, our pharmacists at Ultima Pharmacy are authorized to assess mild to moderate acne and prescribe effective treatments.

Depending on the type of acne you have, we can prescribe prescription-strength topical retinoids (like adapalene or tretinoin) to unclog pores and prevent new ones from forming, or topical antibiotics (like clindamycin) combined with benzoyl peroxide to kill the acne-causing bacteria and reduce severe inflammation.
